1 - The ONLY way to get it is sex. Oral sex doesn't transmit chlamydia, and you won't get it from unclean places. I'm sure his doctor was trying to be helpful, but really - that's foolish.
Since you were both tested recently, and had negative results, if you all have chlamydia, this is a new infection for you both, and would have to come from one of you going outside the relationship.
2 - Yes, lots of other things cause similar symptoms - other stds, prostate problems, urinary tract infections. Its hard to say exactly because you don't mention what his symptoms are/were.
3 - No, you both tested negative since last year. If either of you were infected, it would have shown on those tests. And yes, symptoms typically show within a few weeks if you are going to get symptoms.
4 - No, its not 100% - nothing is - but its more likely than not that if he had it, you'd have it.
5 - They are very reliable, but if you took the tests while on antibiotics, the tests won't mean a thing. They can be falsely negative or falsely positive. The antibiotics might inhibit the growth of the bacteria enough to give a false negative, or the test might pick up on dead chlamydia DNA that can give you a false positive.

the symptoms were mainly itching, like an allergy, and a bit of fever...he says it doesnt hurt.
And you are right - its terrible that he wasn't tested.
