what does mild flattening of hemidiaphragms and mild chronic appearing interstitial changes are present

Mild flattening of the hemi diaphragms is an indirect sign of hyper (over) inflated lungs, the usual cause of which is asthma or COPD.  Mild interstitial changes is a sign of lung scarring.  Neither means that you have serious lung disease.  The status of your lungs would best be determined by pulmonary function testing.
