Chronic Pain in Spermatic Cord

Seen 4 urologists; no resolution yet. Am 23 & for 6.5 months have had severe spermatic cord pain that refers out to the epididymis & lower abdomen. Had surgery 7 months ago to resolve reoccurring episodes of testicular torsion in right testicle. In surgery they didn’t find the deformity that would cause torsion but they removed inflamed appendix testicular & anchored testicle. 5 weeks post surgery moved furniture but felt fine. Nxt day I felt intense pain in & around right testicle. Was not the torsion pain I had surgery for, is new pain (A). Saw new urologist who tested for infection, hernia, had thyroid test, scrotal sonogram, abdominal CT, MRI, Xrays. everything normal. Gave me testicular block shot, worked for 3 hours next morning couldn’t walk. Had a new severe pain at base of penis extending slightly into lower abdomen & testicle. New pain (B) is along upper end of spermatic cord.  Saw another urologist who performed thorough physical to determine specific locale of pain A.it originates in spermatic cord an inch above upper epididymis but he could feel no abnormality. Put me on amitriptyline for past 2 months & noticed little improvement.  Things that aggravate pain A & B: walking quick lower body movements repeatedly getting up & sitting down in a seat (ex. at work) vibration car ride wearing constraining underwear.  Pain A ranges from being sharp to dull aching radiating up & down. Also, whenever Pain A is severe I experience pins & needles on bottom of right foot. has regularly occurred since Pain A started.  No doctor has explanation.  Current urologist “wait-&-see” approach with the amitriptyline isn’t reassuring since it has minimally reduced pain after 2 months.  Don’t want to lose my right testicle & been told removing it might not solve problem. 7+ mo & No end in sight. Need suggestions, doctor recommendations, see neurologist? Willing to travel anywhere try any medication &/or undergo any surgery that doesn’t involve the removal of my right testicle.
