Complications post colectomy with ileoanal pull through

by Drained, Oct 07, 2008 02:08PM
Last November I had a total colectomy with an ileoanal pull through for ulcerative colitis. The pouch was created during the first surgery but I had a colostomy for a few months while it all healed. In February then I had the colostomy reversed. Since then, I have had multiple complications. At first there was a great deal of pain with bowel movements. That has finally subsided however, I currently have 12 -20 bowel movements a day with several occuring at night. I also have a lot of trouble with incontinence. My surgeon has examined me and determined that my sphincter tone is perfect. He seems to be at a loss as to how to help me. I've experimented with antibiotics, diet changes, acupuncture.... the list goes on. I take immodium and acidophilus daily but nothing seems to help. Are there dietary restrictions that I should follow? Any foods or beverages I should avoid? Is there any hope that I won't be incontinent for the rest of my life?

Hi,
It could be due to pouchitis which is a known complication following IPAA surgery.
Metronidazole for about 4 weeks is very effective. Consult your doctor for the dose.
If it is not effective, then other medications like steroids, mesalamine  and bismuth subsalicylate may be tried.

I had this surgery 15 years ago.  It does get easier with time.  Good things to eat that will bulk up your stool are marshmallows, oatmeal, and apple sauce.  Watch how much fiber you take in.  I can still only manage a small salad every other day.  Eat your vegetables cooked.  Try and increase your protein - eat eggs and more meat.  Avoid sugar - it will loosen your stools.  Too much caffiene should be avoided.

I had this surgery 21 yrs ago (@ 18 yrs old). I've had pouchitis twice. I still have trouble w/high fiber foods, anything spicy  & I've become lactose intolerant. My diet is very restricted. I have 6-8 bowel movements a day, sometimes more.
