Pre-Prepared special diet food delivery

Does anybody know of a place where you can order prepared foods that are for special medical conditions? For instance, my sister needs some food that is high in sodium and low in fiber and fat. She has very low blood pressure, impaired ablity to retain enough water to keep her blood volume up, and recently had her entire colon removed. She isn't well enough to cook because she will pass out standing up, and so she needs some already made food to get delivered that can just be warmed up.
