anal fistula

i have been diagnosed with an anal fistula. i live in gas city indiana. there are no colin rectal dr here i have 2 goto indianapolis to see dr george he is at wishard hospital. he has operated 3 times when i was insured. i no longer have insurance. he put the seton in and it did not work. tha was a year ago. the pain is more frequent and most of the time unbearable. what can i do with no insurance, the hospital offers assistance to marion co. residence but i live 65 miles south and because of that they want payment upfront i cant afford it. the county i live in dosen't have a colin rectal specialist and i need to get this fixed i've been dealing with this since 2006. what options do i have? do i have rights without insurance? please help me
