testical sweeling from 20yers

hi i am 26 years old, i am suffering from right testicular swelling from past 20 years, may be rite from the birth, till 2001 it never pained to me, but in next 3 months i had gone through  worst pain in my life, it was so painful that i need to raise my right leg to 90 degree to get rid of that pain. now in 2009 i feel very lil pain in my right testicle ,its size huge as always 2x to 5x then normal and it keeps on changing with time and after intercourse it reduces to great extent. plz put light on to this problem , what it can be
