Toothaches are breaking through my gums. I find they're quite sharp and I've been getting quite a few sores on my

Toothaches together my inner cheek catches slightly between my new teeth. I suspect my right bottom wisdom is also coming in at a 45 angle rather than straight up, pushing the tooth next to it up to a spike, which is also cutting my tongue. My top teeth feel fine, straight and what I presume to be normal, albeit the ones furthest back are a little sharp. I assume these new teeth will grind down slightly with use eating.
My bottom teeth are the real problem, I have over crowding mainly at the front. If you take your fingers and push them together so your middle finger is at the front, the two next to it go slightly behind it and your pinky is behind that - it's something like what my front teeth actually look like. They over lap slightly and I find I need to floss and get a scale and polish every 6 months to keep plaque from building up. My front teeth also lean back slightly so when I put my teeth down together my upper teeth go over my lower teeth, the lower teeth meet my front upper 2 teeth (the big ones) around 3mm before my gum. I don't mind this, it's meant my smile has mainly been my upper 'good' teeth and you can't see my over crowding unless you look while I'm speaking.
Anyway, ever since I turned 17 and was no longer seen by the NHS child dentist, I've not been to see a dentist. I know I'm getting plaque build up but I can't pull myself to go see one. I'm considering asking for teeth to be pulled and a brace to be put in. Would this be wise? Or possible? I'd like my bottom teeth to straighten up and hopefully it'll sort the sharpness at the back. I'm quite scared to go to a 'on the highstreet' dentist. I'm considering contacting my hospital or GP instead. Thoughts? Thanks.
