Wisdom teeth extraction

I had all 4 of my wisdom teeth pulled on Aug 10. 3 weeks later I got an infection that caused my entire face to swell up. I went back and the dentist blamed me for not rinsing or doing this or that. He gave me antibiotics and they took the swelling away but my gums are still inflamed. I went back and he gave me some other kind of antibiotics which did nothing. I haven’t been back since because he said if the antibiotics don't clear up the infection then he will have to perform a second surgery to clean all the infected tissue. In my bottom left hole I've always felt some roughness but I figured it was just the infected tissue. Last night, 2 1/2 months later, I look at the hole in my bottom left and I see this white thing. I pull at it and it comes loose. Then I pull it completely out and begin to bleed. When I look at it, it looks like a piece of tooth. He never told me he left anything in there, but I've read online that sometimes they leave the root in. Could this be the case?
