Can pain be fabricated in your mind to a point where you actually believe you are in excruciating pain even though you are on more than enough pain medication.

Short answer:  Yes.  Longer answer:  Ultimately all pain is subjective.  If you feel it,  you are in pain, but that is a separate question than if you have to be.  We know major surgery with no anesthetic is possible with hypnosis.  Pain has big elements of expectation, habit, focus vs. distraction, and interpretation.  Lots of room to negotiate with your body.  When people had nerve surgery for extreme pain they were asked afterwards, "Is the pain still there?"  Answer "Yes."  Then asked "Does it still hurt?"  Answer, "No."  So the pain and it hurting are two different things as well.  

Sounds like you might be a good candidate for learning some of the psychological/biobehavioral pain control techniques.
