Symptoms of an impending heart attack?

I've been having chest pains, neck and shoulder pain. Nausea, increased sleeplessness and waking every hour. Shortness of breath, dizzyness and, or lightheadedness for over a month now, I went to the ER the night it all began and other than elevated dystolic blood pressure and irregular heart rate, every test was negative. EKG, Chest X-ray and blood enzyme count. It's been getting worse and I have read more than once that these are all symptoms that precede a heart attack in women. I am almost 23 5'9 142Lbs, and I have a pretty bad diet honestly. But there is also a high rate of heart disease on both sides of my family. My great grandmother died at 51 from a massive heart attack, as did her son. My grandfather had a quadruple bypass before he was 50, my mom has had mild heart attacks. My other grandfather has had multiple strokes, my cousin died at 19 when his heart burst from undiagnosed Dilatory Cardiomyopathy. I plan on returning to the ER hopefully tonight but as they were supposed to refer me to a cardiologist and didn't I'm not sure when I will be seeing one.

Wondering if I should be concerned about following echo ~
                        2012.     2014
L~vent
End systolic.   2.8.         3.2
End diast.       4.1.         4.7
L atrium.         3.2.          3.0
Aortic root.      3.2.          2.8
Iv septum.        1.0.         0.8
LV post wall.     1.0.         0.8
Ivs motion.        1.1.         0.9
Lvpw motion.     1.1.        0.9
Epss.                  0.4.       0.4
MVE.                ------         5 (5.0-10.0)
Mitral Ef slope.    98.         240 (50-150mmsec)
Aortic opening.    1.8.        1.7
Quality.             Good.       Good
Pericardial effusion. None.   None
Eject fract.         60.             59
% fraction.      31.                31
RSVP.             29.                30
Tr gradient.      19.              -----
RA est pressure. 10.           ------

Both reports were done is 2-d & all valves grossly normal with normal color & spectral profile & ventricular a, aortic, atrium all normal in size & function
2014 - m-mode data is significant for normal aortic root size, normal left atrial dimension and normal left vent end diastolic dimension ~ should I be concerned that the measurements got smaller ~ what is mitral Ef slope ~ should I be concerned ~ does that number of 240 mean server mitral valve stenosis ~ cause the doctor said my echo was great ~ & the mitral valve was grossly normal with color & spectral profile ~ please help me Iam extremely stressed out! Thanks so much ~ I am 42 year old female ~ normal blood pressure!

Hello and welcome to the forum.

I am sorry to hear of the symptoms you are struggling with.

The symptoms you describe are nonspecific and you are young to be having heart disease.  Having said that, you clearly have a strong family history.  I agree with an evaluation by a cardiologist.  I would recommend and echocardiogram to ensure your heart function is strong and consider a holter monitor which will determine what your heart rhythm is when you are having those symptoms at home.

Wishing you only the best.

Dr. Rich
