Lump in chest after gynecomastia surgery

My son got gynecomastia surgery back in the first week of Dec.  Three weeks later we notice a large lump, the size around 1" wide on his right side right under his nipple and a small one on the other side," this one is not that noticeable'.
The Dr. put me another appointment in March to fix this problem. I asked him why this happened, His response was,"That we were supposse to massage the area and that some patience have these complication".  He still wants me to keep massaging it with hopes that it will break down and the the body will desolve it. I would like to know what exactly is the lump made of and how it was formed?  Is this a serious complication? What other procedures will my son have to go through? Will massaging it correct this at all?
This Dr. performed my eyelid surgery and I was happy with the outcome. My son is 17 years old had gland tissue and fat removed with only liposuction. I asked him if he need to make an incision, he told me" he had studied back in France and that they do  not do that procedure over there" ,they take care of it with lipo.  Could you please give me your feed back on this?

In general I usually perform a combination of both direct surgical resection of dense gynecomastia along with some liposuction to obtain the best and most predictable results.  If the breast tissue feels primarily fatty, then liposuction alone could control the fullness.  The lump your son developed probably is secondary to a local bleed into the tissue within a liposuction tract.  A clot that then occurs is ultimately changed to scar tissue.  Over time (with or without massage), this can disppear with resorption of the scar tissue.  This may take three or four months.  Lupiness following liposuction is a typical finding and usually always resolves.
