Gynecomastia gone wrong

I had breast reduction surgery @ 21y/o of age but it was done badly. My left side chest has more breast tissue forming & the right side is a total crater where breast & muscle tissue (by mistake) were removed. I'm now 37y/o & am tired of being body conscience still (since 15y/o). I can't take my shirt off in public & my sex life is non existent, I get paranoid about what people will think which causes me erectile disfunction.  I'm wondering if there is any type of reconstructive surgery (implant) available to reconstruct my chest. I have an endomorphic body structure so I don't have any definition but I keep fit & swim a lot to try & stay in some sort of shape. I'm 6ft 1 & weigh 170lbs (75kgs). Creating more symmetry is possible with a combination of fat grafting to the caved in side (so-called dish deformity) and removal of residual extra tissue on the side with excess fullness.  An experienced knowledgable plastic surgeon can help you.
