Saving an implant in the presence of an infection can be very difficult and unpredictable. I have had the experience of saving implants in the presence of infection when drainage occurs.  However, if the infection persists, then your plastic surgeon is correct.  The implant will need to be removed.  It may be akin to having an infected splinter.  All the antibiotics in the world may not necessarily cure the infection until the foreign body (splinter, or in your case implant) is removed.
