Hi,
I am going in for a revision rhinoplasty and at all consultations, the Dr.'s said I could benefit from a platysmal plication.
I'm 41.  I don't have a lot of fat or sagging skin.  Genetically, I just never had a defined neck or jaw.  
It seems like there are a lot of complications from this procedure.  One Dr. said he would go behind the ears, and another said he would make an incision under my chin.
I don't want to mess my neck up.  If the risks are high, I won't do it.

While platysma plication without significant skin adjustment is a recognized procedure, the decision to undergo treatment depends partly on your concern about undergoing this procedure.  Its success will also depend on your neck anatomy (i.e. hyoid bone position and the status of your platysma muscles, digastric muscles and subplatysmal fat).  Complications of potential scarring beneath the skin with irregularity and skin adhesion are possible.  An alternative is waiting until you are a proper candidate for a lower face/neck lift which would allow both control of the underlying playtsma muscle and the skin.
