One month ago, I awoke to find my right cheek from eye socket to chin swollen to the size of a softball. I had no previous swelling or pain in the area, I went to the ER, where a CT scan and blood work was done, and was sent to an on call surgeon whom happened to be a plastic surgeon. Upon arrival, he decided to lance the "cyst" from the inside of my mouth, doing a nerve block to subside pain, and he did remove a lot of keritanized matter from the cyst, but not all. I was sent home, with idoform packing and antibiotics and to return in three days. The swelling subsided by day two, and was relatively painless(rather numb), but draining, in my mouth periodically as expected but still gross, I returned to the the MD and he was not "happy" with the result, he stated he could still feel something structural inside the cheek wall between dermis and muscle of my cheek, he requested I return in a week. At one week post-lance, the area of my cheek was beginning to swell again:(
The doctor scheduled a full surgery to excise the "cyst", externally on my cheek. Surgery was successful, to a point, I have very little anwsers from the doctor, I want to know the epidemiology and all the possible causes of the keritanized, sebaccous cyst that had finger like growths all over the right side of my face, with substantial nerve involvement. During the surgery, the doctor performed a nerve block, I am having complication with facial paralysis, i have about 4mm of scleral show on my right eye below my iris and numbness and tingling from the side of my nose to my cheek bone to my jawline, and cannot control the right corner of my mouth, which affects my speech and ability to manuever or manipulate my facial muscles, as well as chew effectively. I was originally told that the numbness should subside in 4 weeks. The numbness would be welcomed instead of the tingling, crawling and electrical shocks and twinges I am now suffering with at this time. Realistically I want to know if a diagnosis can be determined as to what the mass/growth was and how long the paralysis will last in my face. Thank you for the time and attention.
As mentioned earlier also, sebaceous cyst is the commonest to occur on face and contains keratin. Blocked sebaceous glands, swollen hair follicles and excessive testosterone production will cause such cysts.
Generally no treatment is required but they become painful then it means they are infected,Then surgical excision by traditional wide excision, minimal excision, and punch biopsy excision may be required.
It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. First, thank you for the response. I have returned to the MD, and have had no true resolution. I am have lots of neural pain in the area in and around the incision. It seems as though, I have triggers from cold(and spring wont get here soon enough), and if I purse my lips, I have electrical pain, that run from the inside of my righy eye socket, down the right side of my nose, and drops down to the nasolabial fold, and into the corner of my mouth, my speech is still slurred as I cannot control the right corner and right upper lip area..Sometimes the pain is strong, and sometimes it is crawling sensation, and does not stop until i apply light pressure, as soon as I release it starts all over again in the same sequence. I feel like I am developing some type of sensativity to the matter, I am fearful that people are looking at me as I try to stop the constant sensations that never stops, night nor day. The MD, seems to think that because i had deep nerve involvement that it could be causing the sensation but I dont know what else to explain to him that the issue is not resolved and causing issues for me. I am a college student, finishing my degree and have a class in Humanities where I have speeches and presentations and yesterday, my professor asked me to take an imcomplete and not try to continue the class at this time. I am to graduate in June, and if i have to take an imcomplete, I am at the mercy of my professor to grade and post all my grades and work, during summer, and that is a slow process, and I am not looking for to it at this time. I had a stomach virus this past weekend, and after a particularaly bad episode of heaving, my face was numb for hours and finally came back to twingy fabulousness on Monday. Its just scary not knowing what is going on, as the surgery was to supposed to correct the issue, but had made more different ailments. I am trying to stay positive and wait it out, but I am being seriously impacted.
