Should I be concerned about new blood in right arm?

I'm a 52 yr old male recovering from a PE (pulmonary embolism) & a DVT in my right calf from July. I've been on xarelto for 5 months because the DVT has not resolved itself. Yesterday evening I began experiencing severe vein pain/discomfort in my right forearm. The pain is similar to that of having an IV in your arm that becomes painful after several days. This vein pain has been constant and persistent for 20 hours. Never had high blood pressure or any heart/vein related health issues.

Where at in your forearm because I've had arm clots, if you describe your pain I may be able to help, sorry this is late!
