obstructive hydrocephalus

Hi.  My husband had a cerebrovascular accident at 10 days old in 1965.  A shunt was put in.  There have never been any revisions.  He has obstructive hydrocehpalus.  He has a 19mm x 17mm cavernous angioma in his fourth ventricle, a arachnoid cyst mildly deviating the brainstem to the right.  He had a third ventriculostomy in Ddecember of 2005.   After surgery he could not walk or talk.  He can now walk, but falls frequently.  His speech has improved, exept for when he is tired which is often.  He needs to rest alot.  When he is upright for to long he says he feel like he has "cobwebs" in his head.  His latest CT says that there is focal encephalomalacia of the left frontal lobe.  What does this mean?  He says that he will not go through another surgery, and I respect his decision, but what can be expected in the future?  He is by the way 42 years old.  It is very difficult to see the person you love deteriorate in front of you.  Thank you for any answers you might be able to give me.
