How to choose a General Dentist and Periodontist to Treat Bone Loss

What should I be looking for when getting a second opinion/ picking a new General Dentist and Periodontist?  I have bone loss that needs to be treated by the Perio soon and then ongoing Perio maintenance.  Some info suggests they should be picked as a team and one may refer another. I haven't been able to get personal recommendations.

For instance: Should the Perio have Board Cert and be skilled in all types of bone regeneration?  Conservative treatment plan first? Hospital Affiliation? Years Experience? Should I bring my X-rays ?
