Possible fixes for TMJ?

   My wife is having sever TMJ pain, and both directions we are hearing seem pretty scary, and 2 specialist have 2 very different directions to fix the pain...

    The first TMJ Doctor we talked to is telling us that he wants to move the lower jaw forward, and pull the wisdom teeth to reduce over crowding in both her top and lower jaws.  
     The second TMJ doctor we saw prefers to go a less evasive method, but some of what he wants to do seems a bit too much. (Even if it is less then moving the lower jaw forward...)   She is Japanese, and has over crowding in both upper and lower jaws. And either way we will be removing her 4 wisdom teeth which are partially impacted now... However, the second doctor wants us to remove 2 additional teeth, (6 and 26 I think), add braces for 2 years to fix the hole that would create, and then do both top and bottom Frenulectomies as well....
     The second doctor believes the main problem is that her muscles grew in a position that causes stress on the joints, so we need to take away that stress. The first doctor we saw said that we don't know if the muscle positions is causing the problems, so doing all this manipulation would be for nothing if it doesn't work...

Any third opinions out there?  And what is the TMJ benefit of a Frenulectomy? All I can find on the internet is it helping with braces...

After getting a Cone Beam volume scan, we where given a list of these observations;
The right condyle was reduced in size. The superior surface of the right condyle showed signs of sclerosis, erosion, and flattening while the superior surface of the left condyle showed signs of flattening.  When the mandible was in the closed position the right condyle was located within the center of its fossa while the left condyle was located superior to the center of its fossa. the resultant superior joint space was narrowed.These findings are consistent with degenerative joint disease with the right condyle and remodeling in the left condyle.
