TMJ/TMD

I've had TMJ/TMD for a little over a year. I first visited my dentist, who created a night guard for me - which didn't work. Then, I went to a TMJ specialist, who made a different night guard - which also didn't work. Now I'm in physical therapy . The physical therapist believes my TMJ is caused by my scoliosis, so we've been doing exercises for my spine - which of course, doesn't work. Every day I'm in excruciating pain, and so I use heat packs, ice packs, massaging my jaw, and muscle relaxers and ibuprofen. They all only temporarily make a difference. And by temporary, I mean for about 5 minutes. I've heard that the surgery does not have a high success rate, so I'm hesitant to try it. But like I said, I'm in so much pain. Also, my jaw locks pretty much every time I move my mouth (even just a little bit) and I can't even get a yawn out. What do I do next?

Also, I need relief in the next couple of months, as I need to get my wisdom teeth removed, which my doctor said he will not do so long as my jaw pain is this severe.
