What questions should I ask pertaining to a root amputation?
I had an absess upper tooth and had to have a root canal.  The the root embedded in the calcified canal.  The dentist sent me to an endontist and after 2 hours and 2 drill bits later, the endontist said he couldn't get the temp crown off and remarked that he should have been to my pituitary gland by now!  Now I have to go back for a root amputation. What are some of the problems associated with this and what kinds of questions should I be asking?  My father died of Von Willebrand's disease after a fall.  Should the doctor know of this since it is in my family?

Yes, this should be mentioned to your doctor PRIOR to your surgery, not the day of.
