Three months ago I went to a dentist looking for a more attractive smile.  I had no pain, just a mouth full of stained and filled teeth.  I foolishly agreed to a very high price and have ended up with Lumineers on my front six top teeth and ten new crowns. Starting with the temporaries, I have been in pain and discomfort and living on Advil or Vicadin the entire time.  Correcting the bite from one crown is one thing.  How do I go about getting a mouth full of pain fixed?  All of the premolars ache, (I only have four since having four removed prior to orthadontia many years ago) including two that the dentist did root canals on to fix the pain.  I cannot chew on my right side because of the pain from the premolars and cannot chew anything like raw vegetables or apples on my left side without pain from those premolars.  I wake up in the middle of the night with pain in both premolars on the right side that hurt even more since having root canals.
The dentist has adjusted the teeth on the right side, and my bite seems okay to him,it seems to slide around and force my jaw too far back.  When I wake up at night, the bite seems off, the problem teeth making contact and the teeth aching.  The pain returns throughout the day.  
How do I go about fixing a bite that now seems entirely skewed?  Headaches are beginning to pop up making me think TMJ is becoming a problem.  I have had a jaw "click" ever since orthadontia 25 years ago, but never any pain.
Should my dentist be charging me additional money to fix these problems?
