excruitiating, burning ear pain

I have been having a severe, burning pain in my left ear for several years.  It seems to be more often in the last
month or two.  It comes on with no warning.  It is sooo bad I cannot touch it or lie on it.  It lasts for a few minutes up
to several hours.  I use Tylenol for the pain, sometimes it helps sometimes it do not.  I have talked with the dentist
and he gave me a retainer to wear at bedtime.  He says it could be TMJ and clenching or grinding my teeth could
be the problem.  I also went to my doctor who says its an infection. Obviously it is not because I still have it. Each time I am checked for this they say there is no visible sign for this to be happening.  I have been faithful wearing my
appliance and that will be for over a year now.  I even tried acupuncture to help with the pain.  I have to say I am
getting a little concerned as to why it is persisting.  I have read several of the other questions and they seem to
lean toward wisdom teeth.  If that were so wouldn't my dentist know this.   I must say this is a very painful
problem and I wish I knew what it was.  If you have any idea what this could be I would appreciate it if you could
fill me in.

What other specialists have you seen?  Have you tried an ENT?  Or an orofacial pain specialist?  Or a neurologist?
