still pain 3 wks after fillings

I have 8 bottom front teeth left and 3 weeks ago I had 2 filled....one was deep. As the numbing wore off I have had alot of pain. On and off but severe to where it wakes me up and keeps me up! It also comes and goes all day regardless of eating with that tooth or not. The pain is radiating into my ear and now starting today the tooth hurts alot to touch, tap or brush it. Also as of today it is the same pain but a sorta pressure feeling to. 2 months ago I had several extractions and they got infected and I got ear infection at the same time and both cleared up with antibiotics. Could this be another infection or a need for root canal? Should I see my md 1st and get meds incase of infection and wait till after to see my dentist? When I do see the dentist should he xray 1st before just saying I need a root canal? Thank you so much. Oh when I had the fillings he said my gums are badly infected and he cleaned my teeth before the fillings to.
