white dots in corners of mouth

Hello,
About a month ago, my friend felt soreness in the corners of his mouth.  I looked closely and discovered tiny white dots on both sides.  Over the next couple of days, some seemed to group together and become raised.  He went to a dermatologist last week who told him he has angular chelitis and gave him stectazole cream.  After researching, I really don't think that's what it is.  I've seen nothing about white dots relating to that condition.  What do you think this could be?  My friend is not in a high risk group for anything.  (Is spectazole acceptable for treating the mouth areas?)

Usually angular chelitis presents itself as red cracks in the corner of the mouth. If the medication is not effective then a consult with an oral surgeon may be in order.
