I think it is normal that some people are more gifted in a particular area.
I would strongly encourage you to do what feels right for you. If that means study (and get good grades) then that is what I would do.
There will always be someone either more or less intelligent (or gifted) than yourself.
I would strongly urge you to focus on your strengths and what you can control. (That does not mean you are not able to help others who are struggling because if you choose to you can. It is better to help them understand the subject than to sabotage your own efforts -which helps nobody. A higher grade will not mean that they understand the material any more).
I think you should use yourself as a reference and not concern yourself so much by other peoples limits.
Instead of feeling guilty for not studying, perhaps you could put some of that energy into pursuing other interests of yours.
Perhaps a more balanced life would help??
Perhaps your interests, goals and values are changing??
Take a break, do something fun and enjoyable, then go back to the books.
Perhaps if you focused more on the short-terms goals you need to achieve you may not feel so overwhelmed. Focus on what you need to do now, not on the results.
The results will take care of themselves -they always do.
It is OK to be passionate and driven by something. We all have to prioritize things in our life. You can still be self-directed and be respectful of others at the same time.

Go rent the movie "Patch Adams". If you go his route (not that you have to become a doctor to do it) you could be the top of your class without effort and then pay back society in spades!!! Lucky, lucky you.
