Pristiq - side effects & discontinuing questions

I have been on 50mg Pristiq since November 2010.  It has done wonders for my depression and lethargy; however, I am now having what I think are side effects that are least desirable.

Daily headaches and major agitation which I would probably classify more along the lines of anger for no reason.  I've maybe had one or two anxiety attacks since I quit taking Klonopin about 3 months ago but I continue to take Trazodone (50mg) at night before bed to help me sleep.  If I don't have the Trazodone, I don't sleep.

I know there were times where I've missed a dose of Pristiq and within 12 hours of the overdue dose, I get what people refer to as brain zaps or almost unbearable dizziness.

I want off of this medication.  I cannot get into a reputable doctor until the end of October (made the appointment in July) and I need to know what is the best way to slowly stop taking this medication.  I know it's a time released pill, so breaking it in half is not an option.  Has anyone stopped taking this without replacing it with something else and how did you go about doing so?

Also...I realize there are many discussions on this medication but they seem to be older in date so I'm hoping maybe there's more prevalent information available.
