When will it help???

My wife has been taking effexor, she has terrible days where she just wants to cry and hurt herself by banging her head. The effexor hasn't been working so her doctor switched her to cymbalta four days ago, there has been no change, she seems to be getting worse, today was really bad, she said she is tired of fighting it and is ready to give up. How long will it take for the cynbalta to start working? It just hurts me so bad seeing her like this everyday knowing there's nothing I can do to help.

She has taken the effexor about a year, the feelings she has to want to hurt herself and cry all the time has just started the past couple of months.
