Why my cognitive abilities deteriorated.

Hello,first time poster here.
I am 27 years old.I have been diagnosed with depression and social phobia from a psychiatrist.Initially i took escitalopram and olanzapine for one year but then i changed doctor because i didn't like him so much.I noticed that after the first couple of weeks  of taking the meds my cognitive abilities dropped down  a lot.The new doctor prescribed me prozac and rhisperdal but right now i also have short term memory problems.I talked to him about it and he started asking me questions like "what day is it" ,"what;s your name" "start subtracting from 100 7s" and such.I was really freaked out.He told me i don't see anything now ,but we will re-examine this in the future.I am really really concerned if it is something serious like dementia or similar. Can you shed some light?I am having an appointment in a couple weeks and i am terrified.

Tell me something, did you slowly taper off the drugs before changing them or did you stop abruptly?  This could be a withdrawal symptom.

I stopped them abruptly.
