I am a 51 year old truck driver living in NH and have had a similar problem for many years with little success as well in solving it. Itching behind my knees in addition to between fingers and toes and scalp. I was checked by my doctor and prescribed Lotrizone for behind my knees, it did help but has not solved the problem. I did not have lice and he said I did not have scabies either. I sometimes run a hot bath and soak in the tub which helps when Im having trouble sleeping. There is some homeopathic bee-cream moisturizer at Rite-Aid that smells real good and helps somewhat as well I apply after soaking. I know I stayed with my mom for ten weeks early this year in Iowa and was surprised I didnt have any noticeable trouble sleeping or really with itching. She prepares low salt,low fat meals and I was eating smaller more regularly there. Used/drank city water there ... well water here. I was thinking it might be something in my house, I do have a cat and have had for many years recently got a dog and she goes in the truck with me. I have problems in the truck sleeping that are intense and as Im over the road I sleep in it far more often than in my house when Im working. I have a limited amount of time for sleep working and this creates a huge problem. Tried many lotions with a Papaya one from odd lots that seemed miraculous but which they naturally stopped selling after my first bottle ran out. I take goodies PM's to sleep and they make me sleep well if I get the itching calmed somewhat with lotion. It seems as if I am dead tired rather than just needing to sleep I dont have a problem with the itching.

I am guessing the dog you recently got has been to the vet and checked out.  Have you had an allergy test done to see if you were allergic to the dog?  I doubt that you are, but it doesn't hurt to check.  

You can also try using a mild dandruff shampoo to help control the itching on the scalp.  

To control the itching, try zyrtec or claritin 24 hour.  Zyrtec can make you drowsy, but Claritin 24 hour will not.  Aveeno products are also great at controlling itch if you prefer topical instead of oral.
