Acute penile pruritic 1mm pustules

Hello I have been dating a geat guy for about two months and we have had unprotected sex, because I actually would like to get pregnant. I have been tested for just about everything since I am officially infertile and may need IVF, he was tested for just about everything two years ago because he had a red enlarged painful penis that finally resoved with claritin, and has been clear for two years. Within the last week and a half he has now gotten what started as tiny 1mm red raised ares around the rim and dorsal part of his penis. He says that during sex they become itchy to the point of painful and have now sarted to become sort of 1mm pustules. I think he is not circumsized because his skin covers the body and head of the penis when he is not erect. He said he had gotten this a couple of times before but it went away when he bathed. We have been pretty sexually active and hygiene was forgotten a few times and spent all day without washing. He is now washing and applying a triple anibiotic ointment. He is concerned that I may have had something but again I have been checked for lots of stuff etc. What could it be and would a cortisone cream be OK or would over the counter claritin be OK? Any sexual things we should avoid?I do not see him going to the doctor unless it becomes really bad.

It doesn't sound sexually-transmitted to me--more likely the bumps are irritated by sex. I think hydrocortisone is a good dea, plus gentle cleansing and lubrication before and after sex. If it keeps bothering him despite these measures, he should see a doctor.
