After 3rd course of accutane now what?

I started my first course of accutane when I was about 14. It went well but after a few months past it came back again. When  the doctor prescribed the second treatment my skin had severe outbreaks. We had a to do a dose of steroids to calm my skin down and find a new appropriate dosage. The rest of my second course went fine until it came back again. I then at this time went to seek a second opinion with someone that treats more sever cases of acne. He gave me some topicals I used to take before being on accutane because sometimes they work after a course. This was a never ending battle and we came down to the decision of taking another cycle of accutane. We did it and yet again my skin reacted with severe outbreaks and we kept continuing to drop the dosage until we got down to 5mg a day. Previously i started out with 10mg/day and worked my up to 60mg/day. So I took low dose accutane for a year and now  I have been off of it for a little over a month. Sadly I am started to get more and more little bumps everyday and don't know what to do anymore. Any ideas?...because I am going off to college soon and its very hard to get an appointment with my doctor because he is never in!! And the college I am going to is on the other side of the U.S. , Arizona. Do you know of any good dermatologist in AZ that deal with extreme cases?
