I am using bacitracin and neomycin + metronidazole (400mg 2x daily) I hope that works and I will wash with warm water as you suggested. I'm thinking of adding cream clindamycin (eg Dalacin) and will be using the hydrocortisone cream 1% do you think?

From the picture,it looks like balanitis. Oral course of antibiotics is needed.In addition,pls don’t use any cosmetics on the penis,instead use some topical antimicrobials like Bacitracin or Lotrimin.Also corticosteroids and topical immunosuppressants can be used but they are available under prescription. Also retract the foreskin daily and soak in warm water to clean penis and foreskin.In case the symptoms persist then pls consult a urologist/dermatologist for evaluation and antimicrobials and steroids prescription.

Without examination,confirmation of a diagnosis is tough but it can be folliculitis,contact dermatitis,rash due to friction rub with clothes, dermatitis (atopic, contact, allergic),excessive sweating causing sweat dermatitis or balanitis(due to an underlying infection (bacterial and fungal as well as other organisms) .
You may try calamine lotion on the rash.You should also maintain hygiene and try to keep the area clean and dry, wear loose-fitting cotton clothing. Also use an antibacterial soap or mild soap like dove.Avoid any kind of cosmetics.You can also use a barrier cream with zinc oxide paste or calamine lotion or a mild steroid cream like dermacort on the rash.
If the symptoms persist then it will be best to get it evaluated from a dermatologist.
