Breakout in 4 month old

My friends baby is four months old and a few days ago broke out in a really bad rash. They are circular and pretty large. She brought him to the doctor, and they kind of brushed it off. They changed his formula and gave her some cream, but she says its not doing much. He tries to scratch at them a lot. They are everywhere-on the back of his head, his chest, his back. Does anyone have any idea what this may be? He did have excema since two months old. When that went away, it was like 2 days then this appeared.
