Candida antigen injection treatment

Does anyone know of any lists or references for medical institutions, physicians or dermatology practices who do the Candida antigen injection therapy to treat warts?

I am in the southwest US, and dermatologists locally refuse to do the injection therapy, which means I'm stuck with freezing, scarring and recurrence.  I had injection therapy long ago while I was in college (far away), and am now looking for a dermatologist, or practice group or medical institution that does the injection therapy.
