Constant Scalp Itch and Hair Loss!

Dear Doctor,
I am an 18 year old female and pestcatarian. About three years ago, I strated losing large amounts of hair (more than the normal 100). In the shower, at least 60 hairs fall out. When I brush my hair more hair falls out. I see my hair on the floor and on the back of my clothing (everywhere). Last year, I started to constantly scratch my head, so even more hair falls out now. I find that sometimes I will wake up from sleeping and I am scratching the back of my head. I do not have lice, and I use head & shoulders/ dermarest (medicated shampoo that eliminates itch). These shampoos do not work, I still have an itchy head, dandruff  and hair loss. There have been no traumatic events in my life, and I do not take any prescriptions. I take a multivitamin/multimineral everyday.
