Expired Permethrin on Genitals

I am 37 years old. Had protected sex 5 minutes and then condom was removed and hand job completed by her. The next morning for precaution I applied permethrin all over my penis – scrotum and peranial area. Around noon I started to feel a mild burning sensation near tip of urethra. I left the permetrin on for 8 hours and washed off. I did remember the area it was applied burned lightly all day. The burning and mild ache lasted the next 2 days. I was wooried it was an STD. On the 3rd day after exposure went to my GP and he did a urinalysys and STD panel. After 4 days the results came back all negative for STD’s. 2 weeks past and the burning and aching stayed constant inside the penis not urethra cause it never hurt to pee and no discharge. Also scrotum aching and mild burning as well.  so on the 3rd week went to GP and retested again. Again all STD’s are negative.

I went to urologist and he couldn’t find anything and just said its prosatitis. Gave me 1000mg Zithromax and 7 days of Doxy. Started this yesterday. I was losing my mind  to what this could be THEN I remembered the permethrin I did. I went to look at it and noticed the expiration date on it was from 2 years ago! My questions:

- Could not treating this outdated Permethrin cause it to work down into my dermis and cause this constant burngin and aching?

- Also what can I do to treat this if this expired permethrin is deep into the tissue?

- Since this is not an STD this is the ONLY then I did that day in which all this started. Is it possible it was the Permethrin?

Hello,
Permethrin is absorbed in the skin but it cannot cause burning and aching for so long.It sounds like contact dermatitis or balanitis.
Avoid using any lotions and creams for the meantime.Use a mild cleanser with moisturizing properties like Cetaphil. Also use cotton clothes and change the clothes twice if there is any increased sweating.You can also apply calamine lotion or mild steroid like dermacort.

If the symptoms persist then pls discuss the possibility of balanitis with your dermatologist. Prescription of topical and oral antifungals may be needed.

Could anything that exposed me that night like the Nonoxnol9 in condom or spermicide or the dial antibactial soap I used cause to clean with cause this extended irritation? Its been 6 weeks and miserable. Is there any OTC creams I can use on my penis and scrotum to help? Maybe theres fungi thats causing this. My Derm appt is not until 2 weeks and would like to try something.

Hello,

For balanitis, pls don’t use any cosmetics on the penis,instead use some topical antimicrobials like Bacitracin or Lotrimin.Also corticosteroids and topical immunosuppressants can be used but they are available under prescription. Also retract the foreskin daily and soak in warm water to clean penis and foreskin.In case the symptoms persist then pls consult a urologist/dermatologist for evaluation and antimicrobials and steroids prescription.
