Getting over fear to visist Docotor

Four years ago my general practitioner diagnosed me with having a condition. When he diagnosed the condition, he became turnoff from helping me and since I have developed a fear of seeing a doctor.

Question: How do you get over this fear? And how do you schedule an appointment with the office personnel to make sure you get enough time to go over all this with the derm? It embarrassing topic to discuss when trying an appointment over the phone. Words of wisdom needed. Thanks!
