I really never have had a bad acne problem. I would get an occasional bump or two usually around a har. When the hair was plucked, the bump would go away. I have occasionally had some clogged pores and cysts, but it was not an every day or even monthly occurance. The cycts were sebaceousBirthmarks - pigmented Hair follicle sebaceous gland Sebaceous cyst cycts that maybe came once every year. When I was 34 years old an area on my left cheek began to have small pustules which i could clearly see a hair in. It wasnt the classical ingrownIngrown toenail hair, rather a hair that almost broke the skin surface, but never did. Of course a whitehead would or would not form. Sometimes the hair would just stay there below the surface. The area has continued to grow over the past 2 years and I have seen 6 different dermatologists looking for an answer. They all say acne and maybe rosacea. They also commonly mention folliculitis. I have pretty much been persistant with my latest Dermatologist for a year, The treatments I have been given include Evoclin Foam, Tazorac, Ziana, Duac, and a mulitude of oral antibiotics. Nothing has helped. About 6 months ago, a PA suggested that I try laser hair removal in the area. I thought it was a good idea. I had 4 treatments and the two of them produced some bad follicilitis. Teh first seemed to go away within 2 weeks. The second however, has yet to heal. This area of my face is still red and inflamed. I had the treatment in January and it has never gone away. The PA suggested that I see the MD. The MD actually said that it appeared that the deeper hairs had not came out and that I should try a steroid cream to help the inflammation and to help the skin push the ashy particles out. the hairs are coming out little by little in 4 months. However, it is producing bumps similar to acne and scarring is taking place from the lesions it is leaving. The area is red, inflamed, anf the texture of the skin is just not right. The skin in that area of my cheek is more wrinkled and sort of lumpy underneath. Also, the skin is really red in places and I can see a lot of blood vessels. Some of the hairs are really deep nd do not surface. Others are right at the surface yet grow underneath the skin without breaking through. I have asked the derm about fungus, mites, and just about everything. He says that he sees no signs of either. I tried some over the counter antifungal myself and it didnt help. So after a month straight of this steroid (Cutivate) then two weeks on and two off for the remainder of the time, he thinks we should try a low dosage of Accutane (20mg/day). I weight around 155lbs. I have never had any testing done on the skin and no one has ever suggested it. Just as a point of reference, the skin on the rest of my face is relatively clear with an ingrown hair or deep ingrown hair causing a pimple every once in a while mostly on my chin, neck, and occasionally cheek
Is the low dose of Accutane a good idea to try? Also, are the side effects less with this dosage? Have you ever had any experience with a condition like I described? I am unsure whether the accutane will cause the side effects at this dose and, quite frankly, it worries me. I do have sensitive skin and some of the topical retinols dried me out and caused redness and peeling. Even the Evoclin made my face very red because of the alcohol. I do not want ot make it worse, but I would like to regain the clear skin I once had. Can you offer any advice?
