Help! Excessive frightening dandruff case.

Ok, I have a relatively oily scalp and lately I have had a severe dandruff problem. It's not just little white dots or even flakes 1-2mm wide. Occasionally i will find literally chunks of skin 1-1.5 cm in diameter, dotted with dozens of little holes through which my hair poked. These skin patches are brownish and somewhat rigid, and they have the texture of aluminum foil (no joke). My scalp will also frequently bleed. It started after I grew my hair out to 3-4", but then I cut it back to 1/2-1" and it's still there.
For what it's worth, I am a 16-year-old Asian-American male, 6' 0" 195 lbs.
