Hives?

I have suffered for 15 years with "hives" that appear without any life changes, ie, soap, food etc.  They will last 3 weeks or when treated with antihistameans will go away then come back a week or so later.  It feels like there are hundreds of needles pricking me at the same time and is EXTREMELY itchy.  It feel like the itch is under my skin, and I don't always break out.
