Intermittently painful spot on skin - picture included

I've had this spot on my back for 3-4 weeks at minimum. At least that's when I first noticed it. My shirt rubbed it and I felt a sharp pain. It felt fine for a while, but I still feel pain that ranges from sharp to dull every so often. I have a dermatologist visit coming up next week where I will receive a full body screen, but the suspense is killing me. Any derm experts willing to make an educated guess? Much thanks.
