Irritated urethra and red spots on penis.

Dr. Rockoff,

I need your expert opinion on an issue that has been troubling me a lot!   Approximately 3 months ago I had unprotected sex with a friend.  This broke a lot of years of monogamy (which has me depressed, dissapointed and stressed beyond words, but that's for another forum :), I have attempted to return to a normal routine but have been consumed with fear. Here's the deal...

About a month after the incident, during one of my constant thoughts of the incident I realized I could have contracted a std.  Soon after, I noticed 2 irritated red spots on the tip of my penis, in close proximity to the opening of my urethra.  The affected area may even look scaly at differnt angles.  I have sinced checked my self out with every trip to the bathroom.  

In my opinion the only possible alternative to an std was the fact that I had been working out heavily in an effort to deal with my stress.  I was thinking maybe poor hygiene from sweaty clothes or friction from jogging.  So, I stopped the exercise for about a month and still have two red spots on the tip of my penis. They seem to fade and take different shades of red and at random times they seem not to be present. I have not noticed anything I would call a blister and the discomfort is not too bad but gets your attention at times. At it's worst, the best description would be a skin burn/tear because the skin looks sensitive and raw. Urination is pain free but when I produce semen it burns for a minute or so at the end.  

I went to my doctor who took a culture of the inside my urethra (there was no spots, rash, irritation at the time). I also urinated in a cup for tests.  I then went to a urologist, he had me urinate in a cup and said come back when the spots are present.  The tests came back negative. He suggested baby powder for jock itch.  Ironically, when I apply baby powder the red spots actually look more noticable.

This is where you come into play,  I want my normal life back and want to eliminate all probable horrific causes(Urethritis, HPV, Herpes to name a few) of my irritation.  I know I was clean prior to this encounter.  I have asked my friend about her past and she insists she is clean.  She even told me she just had a pap smear that was negative.    

I am going back to my primary doctor I want to ask that more tests be done.  I need to know what exact tests I should request even if no spots, irritation or slightly swollen urethra is present.  What tests would you recommend to adequately cover any possible stds that match my description?

I also would like to add that just recently, after having sex with my spouse she complained of foul smelling urine. Also, I have noticed a wart on my hand. I did use my hands in the episode I mentioned. I am unsure if this is connected.

I eagerly await your response and aplogize for the long post.  I am worried like crazy and you see what worrying did to the grape? Turned him into a raisin!

I can't think of any STD your description reminds me of, nor of any tests which would be relevant.  You've seen doctors, who saw nothing.  I understand your being consumed with guilt.  I think you should get the help you need to deal with that guilt.  What I don't think you need is to rule out an STD, because you have already done so.  The foul-smelling urine and wart are unrelated.

It seems that the tests I have done (urine and culture) would not indicate any of the stds I suspect.  I will follow up with my Doctor.  

If this does not sound like any stds, you would call it a worry spot?  Any additional would be appreciated.

the symptoms just described seem exactly like mine are now and i've been searching about them and these are the closest symptoms. however, i haven't had sex. i've had oral sex, except it didn't happen until a few weeks after the most recent. i don't know what to do. it gets better at times, i'm hoping it will get better and stay better but if not then i will see a doctor. did you ever figure out what it was he had?
