Hi, I have recently moved for work to a very hot climate from the freezing colds of England. I met a girl who I started having unprotected (stupid i know) sexual
relations with this past weekend. Saturday and Sunday I was fine (did feel slightly sore on Sunday but I only thought it was due to performing for so long). After intercourse
under my foreskin that was white in colour so I ensured I washed it, I also read it would be good to wash it with warm salt water as this can help. Since Monday up to now, the tip of my penis
is still hurting, today when I painfully pulled back the foreskin it looked shiny and felt sore. I am a very hygenic person and am always sure to wash my bits properly - especially after sex
. I feel it may have been from the partner....maybe I caught thrush from her? Now I am still in discomfort, I have not seen any reoccurance of the white substance
however the pain is getting very bad. When I pulled my foreskin back today it wouldnt pull over the tip as it usually would, it was like the foreskin was stuck to the tip of my penis, finally when i pulled it back I noticed tiny cuts (2) on the foreskin. I have taken a look into this in great detail on the internet and all the signs point to balanitis. I would really appreiciate your help. I went to the local chemist, I have taken Zithromax and got myself some cream (betamethasone clotrimazole) - it says it is an anti inflammatory and anti fungal cream, not noticing any sudden differences, could you please advise? could you also advise me if this is something serious?

It looks like balanoposthitis. Oral course of antibiotics is needed. In addition, please don’t use any cosmetics on the penis, instead use some topical antimicrobials like Bacitracin or Lotrimin. Also corticosteroids and topical immunosuppressants can be used but they are available under prescription. Also retract the foreskin daily and soak in warm water to clean penis and foreskin. In case the symptoms persist then please consult a urologist/dermatologist for evaluation and antimicrobials and steroids prescription.
