It is difficult confirming anything without examination and investigations, but since the skin tag has been present for such a long time without any symptom and growth so chances of being it a genital wart are less. Moreover PAP
test has come out negative. However if it grows in size and becomes cauliflower like, then the possibility of skin tag has to be evaluated.
