My wife can't take the sun

My wide has always been a sun worshiper, 2 years ago she spent a little too much time out and developed small bumps on her lips and body. Her doctor prescribed some cream and sent her home. Since then anytime she goes into the sun for more than 30 minutes she starts to break out even when she uses sunscreen that covers UVA and UVB rays. Last year she even got what looks like large water blisters and rashes. Does anyone have any idea of what is causing this?

She went to a tanning bed this year to see if she would still break out and she was fine. Even after she got a nice base tan the sun still had the same effect.
