Need Opinion

by worried/nervous, Sep 11, 2006 12:00AM
Hello Dr.

I did something that I greatly regret about 11 weeks ago.  I received unprotected oral sex for about 3-5 minutes from an escort type female.  Other than this encounter, I have been entirely monogamous in my nearly 8 year marriage.

I have not had any typical symptoms of herpes or any other STDs since the encounter, but I have a few questions about some issues that I would appreciate your opinion.

I have a history of jock itch and psoriasis.  The jock itch affects my inner thigh/groin area, the lining of my butt crack, and my buttocks.  The psoriasis affects my scrotum and anal area, among other areas, and makes these areas red and itchy.  Also, I have hemmorhoids.  I have seen doctors in the past who have diagnosed these problems.

As a result of what I believe is jock itch, I have brown blotches/spots on my right butt cheek that has been there for a few months and are about the same color as my inner thighs.  It covers the bottom half of my right buttocks cheek and connects to my butt crack lining and inner thigh.  (My left side usually is also affected, but my right side has been more so lately).  When I have a "jock itch" infection, my inner thigh/groin area, butt crack lining and butt cheek itches very much, presenting defined raised borders in these areas.  Clotramizol and Lamisil seem to work very well, but the brown spots seem to remain.

My questions are:

1.  Seeing that herpes resolves within 2 weeks and based on the fact that the color of my buttocks has remained the same for at least a couple of months (maybe even longer), and based on the description of my symptoms and its resolution using lamisil and clotramizol, is it safe to assume it is jock itch and not herpes?  (I am pretty sure I had the brown spots before my idiotic encounter, but my nervousness has gotten the best of me, and I just wanted to make sure).

2.  When drying off from taking a shower, my 2 year old son snuck up behind me and hugged me and inadvertently "kissed" me in my buttocks on the affected area.  Any danger posed to my son regardless of what the condition is?

3.  I had what I believe was a pimple in between my inner thigh/ groin area.  When I popped it, it popped just like a regular pimple with a white head exploding out and then some blood.  Is there any chance that a herpes blister would pop like a pimple?

4.  I have not had any chancre like lesions.  About 2 days ago, I noticed a small rash (maybe 8 small bumps) on my side midway between my waste and arm pit.  It itches a little and seems to be improving with Eucerin mosturizer.  It is very faint and small.  Any chance something this mild could be secondary stage syphilis?

5.  Based on the very low risk encounter I had, am I wasting my time thinking about this and should let it go?

Thanks so much!

There's nothing in your description that sounds anything like herpes.

Don't think so.

Not likely.

My goodness!  You're thinking about this way too much.

5. Based on the very low risk encounter I had, am I wasting my time thinking about this and should let it go?

I wouldn't say you're wasting your time.  I understand why you're concerned, but the only way you're going to get definitive answers to these questions and get the whole episode behind you is to see a doctor in person, perhaps the one who diagnosed your groin rashes to begin with.

Best.

Doctor,

Thank you for the answers.  I will visit a dermatologist.

Just one overall fallup, from your assessment, are you saying none of this sounds serious to you?

Also, when my son inadvertently "kissed" my fanny, the condition had resolved except for the discoloration.  I know you said you don't think it will harm him, but does this even reinforce your opinion more?

Thanks so much.

Doctor,

I forgot to mention, the "jock itch" gets inflamed and the rash sores (defined border like) get much bigger and sometimes bleeds when I itch too much or too hard.  I wouldn't say it forms a blister, but the rash sores defintitely get inflamed.  They subside after a shower or when I wake up in the morning.  It defintely gets better after I use clotramizol or lamisil.

Does it make any differnce that it gets inflamed and bleeds sometimes.

None of these additional comments makes me any more concerned than before.
