Ongoing Problem

Hi Dr. Rockoff

I am a 26 year old guy who has experiecned acne for half my life, probably even more. I went on accutane 3 times, ages 15, 17 and 20. After that I have tried proactiv and every medication a dermatologist could imagine. Nothing besides accutane ever worked. At this point, I don't see myself growing out of it.

I went to get a facial last week and the aesthetician said that I don't have oily skin and that what I have is not acne but ingrown hairs. Most of it occurs around my mouth and chin, I really don't get much on the sides of my cheek or forehead. She gave me this thing called tend skin but it seemed to just dry my face and I got some more blemishes after using it.

I don't know what to do. I could really use some advice on how to shave. I use an electric razor because the blade was killing my face. It seemed to help for a while but not a ton anymore I guess.Is there any type of laser therapy or alternative treatment I can use?

If what I have is folliculitis and not acne, is the treatment different? Ive heard you talk about Hibiclens, could this help my face? Ive also thought about taking Zinc.

I just need advice and I don't believe minocycline or differin is going to do the trick, I need something different because at age 26 I don't think I am dealing with a typical case of acne or whatever. I don't know any other guys my age with this problem.

What do you think about the Hibiclens and the Zinc?

And could this be acne at 26?

I suggest that you get advice from a physician (in person) rather than an esthetician.  It's possible that you have ingrown hairs.  If you do, laser hair removal might help.  If you tweeze or pulls at your hairs, you need to stop.  If it is acne, there are many medications, oral or topical, to consider.  I don't think you ought to decide in advance what will or won't do the trick.
