Penis Rash

I had sex with my girlfriend 4 months ago.  The sex was rather rough and lasted for several hours.  She preformed oral sex and we had regular intercourse.  She is HSV1 positive (but had not had signs of a cold sore in a year).  I did not use protection because she just had her yearly pap smear and was also checked for STD’s (all negative except HSV1).  

A week after intercourse, I had a discharge and red rash on the head of my penis, but no blisters or ulcers.  I visited the doctors and was tested for chlamydia, gonorrhea and bacteria and treated for chlamydia and gonorrhea just in case.  All tests came back negative.  The discharge went away a week later, but the rash stayed (red rash covering head of penis).  I also started to get a sore throat and had a weak puss filled blister in the back of my mouth around the salvia glands.  I went back to the doctors (concerning my penis, not the throat) and was tested for HSV 1 and 2, and treated with diflucan for a possible yeast infection.  Diflucan did not help the red rash, I went back to the doctors who did not know what it was (HSV blood test came back negative).  My girlfriend went and got tested for the same and had another pap smear, all tests negative except hsv1.  

Two months later, I had an HSV1 outbreak (single blister) above lip, and have had several outbreaks since then above lips.  My question is, I know I now have HSV1 around my mouth, blister are a tell tell sign and I guess the blood test was taken too early.  But I do not know why I still have a rash on the head of my penis.  If it is herpes (HSV1) or irritation, I assume it would have gone away by now (4 months).  It is most red around the tip and top of the head, and the rest of the glans is discolored slightly red.  The rash is just dull red, blotchy at times and slightly irritating (like a minor rug rash, especially on the top of the head and on the tip).  No blisters, just red and irritated skin.  If I stretch the skin, I can see individual red spots on top of the head (blotchy).  

Could this be herpes?  If it is irritation, why has it not gone away?  Could the rough sex have damaged the skin on the head of my penis?  What else could it be, fungal?  It definitely showed up after the intercourse and is extremely annoying.

This could be a condition called balanitis. Balanitis is inflammation of the glans penis. Lack of aeration and irritation because of smegma and discharge surrounding the glans penis causes inflammation and edema.More common in uncircumcised males.

It is usually caused by poor hygiene in uncircumcised men. The inflammation can be due to infection, harsh soaps, or failure to properly rinse soap off while bathing.
One common organism associated with balanitis is a yeast known as Candida albicans.  Balanitis may occur because of excessive growth of Candida, due to moisture and warmth under the foreskin.

It can be treated by applying Miconazole, an anti-fungal medication.Most cases of balanitis can be controlled with medicated creams and good hygiene.

Inflammation has many possible causes, including irritation by environmental substances, physical trauma, and infection by a wide variety of pathogens, including bacteria, virus, or fungus—each of which require a particular treatment.

Retract the foreskin daily and soak in warm water to clean penis and foreskin. Apply bacitracin (not Neosporin) for pediatric patients if bacterial infection is suspected.
Apply topical clotrimazole for adult men with probable candidal balanitis.
